Patient Survey Drawing – Official Rules
NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING. VOID WHERE PROHIBITED BY LAW.
1. Sponsor
Synergy Wellness Group, Inc., doing business as Synergy Wellness Center (“Sponsor”), 7910 Downing Avenue, Suite 100, Bakersfield, California 93308.
2. Eligibility
- Open only to legal residents of the State of California who are 18 years of age or older as of the start of the Entry Period.
- Employees, officers, and directors of Sponsor, and the immediate family members and household members of each, are not eligible.
- Participation in the Drawing is not conditioned on being a current or former patient of Sponsor, on receiving any service or product from Sponsor, or on making any purchase or payment.
3. Entry Period
The Entry Period begins September 1, 2026 at 12:00 a.m. Pacific Time and ends September 14, 2026 at 11:59 p.m. Pacific Time. Entries received outside the Entry Period will not be eligible.
4. How to Enter
- Online survey. Complete Sponsor’s patient survey during the Entry Period and select the option indicating that you wish to be entered in the Drawing. You must provide a valid name, email address, and telephone number.
- Mail-in entry (no purchase or survey completion necessary). Hand-print your full name, complete mailing address, telephone number, email address, and date of birth on a 3″ x 5″ card and mail it in a hand-addressed envelope with sufficient postage to: Patient Survey Drawing, Synergy Wellness Group, Inc., 7910 Downing Avenue, Suite 100, Bakersfield, CA 93308. Mail-in entries must be postmarked by September 14, 2026 and received by September 21, 2026.
- Entries submitted by mail receive the same chance of winning as entries submitted through the online survey.
5. Bonus Entry
- Online survey. Participants who indicate on the survey that they are willing to speak with a Sponsor representative by telephone about their experience will receive one (1) additional entry, for a total of two (2) entries.
- Mail-in entry. Participants entering by mail may receive one (1) additional entry, for a total of two (2) entries, by hand-printing the word INTERVIEW on their entry card.
- Indicating a willingness to be interviewed does not obligate any participant to participate in an interview. Declining an interview after entry will not affect eligibility or the number of entries received.
6. Entry Limit
Limit two (2) total entries per person across all entry methods, regardless of the number of survey responses or mail-in cards submitted. Duplicate or excess entries will be disqualified. Entries generated by script, macro, or any automated means are void.
7. Prizes
- Three (3) prizes will be awarded. Each prize consists of one (1) $500 Visa prepaid gift card.
- Approximate Retail Value (“ARV”) of each prize: $500.00. Total ARV of all prizes: $1,500.00.
- Prizes are not transferable and no substitution or cash equivalent will be provided, except that Sponsor reserves the right to substitute a prize of equal or greater value if the stated prize becomes unavailable.
- Gift cards are subject to the terms and conditions of their issuer. Any costs not expressly stated as included are the responsibility of the winner.
8. Odds of Winning
Odds of winning depend on the total number of eligible entries received during the Entry Period.
9. Winner Selection and Notification
- Three (3) winners will be selected in a random drawing from among all eligible entries on or about September 15, 2026. The drawing will be conducted by Sponsor, whose decisions are final and binding in all matters relating to the Drawing.
- Winners will be notified by email and telephone using the contact information provided with their entry.
- A potential winner who cannot be reached within five (5) business days of the first notification attempt, or who fails to claim the prize within ten (10) business days of notification, forfeits the prize, and an alternate winner may be selected at random from the remaining eligible entries.
10. Taxes
All federal, state, and local taxes on a prize are the sole responsibility of the winner. Prize values are reported as required by applicable law.
11. Privacy and Confidentiality
- Information submitted with an entry will be used by Sponsor to administer the Drawing and, where applicable, in accordance with Sponsor’s Privacy Policy.
- Sponsor will not publicly identify any winner as a patient of Sponsor, and will not disclose any information regarding any individual’s treatment, services received, or health status in connection with this Drawing.
- Acceptance of a prize does not constitute consent to the use of a winner’s name, likeness, image, or statements for advertising or promotional purposes. Any such use will occur only with the separate, specific, written authorization of the individual, which may be withheld or later revoked without affecting the prize.
- Survey responses are confidential and are used by Sponsor to evaluate and improve its programs and services.
12. General Conditions
- By entering, participants agree to be bound by these Official Rules and by the decisions of Sponsor.
- Sponsor is not responsible for entries that are lost, late, misdirected, incomplete, illegible, garbled, or not received for any reason, or for technical, hardware, software, network, or telephone malfunctions of any kind.
- Sponsor reserves the right, in its sole discretion, to cancel, suspend, or modify the Drawing, or any part of it, if fraud, technical failure, or any other factor beyond Sponsor’s reasonable control impairs the integrity or proper functioning of the Drawing. In such event, Sponsor may select winners at random from among all eligible, non-suspect entries received prior to the action taken.
- Any attempt to deliberately damage, undermine, or otherwise interfere with the legitimate operation of the Drawing is a violation of criminal and civil law, and Sponsor reserves the right to seek remedies and damages to the fullest extent permitted by law.
13. Release
By participating, entrants release and hold harmless Sponsor, its affiliates, officers, directors, employees, and agents from any and all liability for any injury, loss, or damage of any kind arising from or in connection with the Drawing or the acceptance, possession, or use of any prize, except where prohibited by law.
14. Governing Law
These Official Rules and all matters arising out of or relating to the Drawing are governed by the laws of the State of California, without regard to its conflict of law provisions. Any dispute shall be resolved exclusively in the state or federal courts located in Kern County, California.
15. Winners List
For the names of the winners, send a self-addressed stamped envelope to the mail-in entry address above, marked “Winners List,” to be received no later than December 31, 2026. Winners will be identified by first name and last initial only.
